Who needs higher vitamin D supplementation?

Prof. William Dalton, Ph.D.The significance of vitamin D becomes crucial, specifically for certain at-risk populations who require additional supplementation. These populations have biological hurdles that impair vitamin D production or absorption. If these populations do not receive higher dosing, they will develop a deficiency faster and could experience negative health repercussions. Furthermore, I have treated many patients who greatly improved their health with customized dosing.
Seniors are particularly at risk of vitamin D deficiency due to the changes associated with aging. For example, skin loses approximately 75% of its ability to convert UV-R (sunlight) into vitamin D after the age of 70. Kidneys also have slower processes, which require an increased intake. I have had nursing home residents who required consistent 4000 IU daily to sustain levels.
Skin Tone & Geography
- Dark skin: Needs 3-6x longer sun exposure
- Northern latitudes: Require year-round supplements
- Full coverage clothing: Mandatory 2000-4000 IU daily
Medical Conditions
- Obesity: Needs 2-3x standard doses
- Bariatric surgery: Requires liquid D3 exclusively
- Malabsorption disorders: Higher doses with fat co-factors
Age Extremes
- Infants: 400 IU drops from birth
- Teens: Increased during growth spurts
- Seniors: 800-4000 IU based on mobility
Cultural practices also cause specific supplemental requirements. Since fabric blocks out UV rays, hijab wearers require year-round supplementation dosing. Orthodox Jewish populations require special considerations as they have unique exposure expectations based on the months. I create individualized plans honoring belief systems while also attending to health promotion.
Testing remains crucial for precise dosing. Initially, high-risk groups should be tested for 25(OH)D every quarter. Optimal protection is achieved whenever your 25(OH)D is in the range of 40-60 ng/mL. Adjust doses seasonally, increasing during the dark winter season. Always do magnesium as well with the vitamin D for better utilization.
